Crisis Hotline/Crisis Services - A continuously available staffed telephone service is available 24-hours per day, seven days per week to provide information, support and referrals to individuals residing in Nueces County. Screenings are conducted either by telephone or face-to-face by a Qualified Mental Health Professional to determine the need for an in-depth assessment. Psychiatric consultation is available based on the seriousness of the crisis. Crisis services are provided to individuals determined to need them through the initial screening. Referrals to services needed are made.Mobile Crisis Outreach Team (MCOT) - Developed to respond to requests for crisis services in the community in a timely and efficient manner, providing the best possible response for individuals in a perceived mental health crisis situation. The team works in close conjunction with the Corpus Christi Department's Crisis Intervention Team (CIT) and Nueces County Sheriff's Department. MCOT is available during peek crisis hours of 2 p.m. to 11 p.m., 7 days a week.

Intake/Assessment/Referral - The first step for adults who wish to receive or who are referred for mental health services, is an assessment by qualified staff. Necessary documentation including written evaluations and treatment plans are completed on new admissions. Service recommendations/ assignment are made considering needs and preferences. Individuals who do not qualify for admission to services are referred to other community resources.

Crisis Respite Services - Are provided at our Pathways home to assist consumers in managing their crisis situation and return to integrated community living as soon as possible. Program goal is to provide support during crisis situation and prevent hospitalization.

Routine Case Management - Routine Case Management is eligible individuals based on diagnosis and needs assessment. Services assist eligible individuals in gaining access to medical, social, educational, and other appropriate services that will help achieve quality of life and community participation. Service activities include crisis treatment, monitoring, screening and assessment, service planning and coordination. Program goal is to maintain stability in the community.

Rehabilitative Case Management - Provides psychiatric rehabilitation services to authorized consumers focusing on empowerment, symptom management, and meeting identified individualized treatment needs. Rehabilitation services include site-based individual and group training and community-based individual training. Program goal is to improve life skills and independent functioning, reduce hospitalizations and arrests. Supported Housing services may also be provided. These service activities are designed to assist persons with severe and persistent mental illness choose, get and keep regular integrated housing. Services consist of individualized assistance in finding and moving into regular, integrated housing; temporary rental assistance and coordination of activities that facilitate access to resources or services that support or assist the consumer. Medication Clinic - Provides psychiatric services to prescribe and maintain medications. Provides medication monitoring and education to better treat and inform the consumers of the need for continuous treatment. Assists in obtaining medications, laboratory studies and physical examinations as ordered by the psychiatrist.

Assertive Community Treatment (ACT) - Program providing intensive community-based rehabilitation services to identified consumers with severe and persistent mental illnesses who have a history of multiple hospitalizations, involvement with the judicial system, homeless shelters, or substance abuse. Staff to consumer ratio is 1:10 with limited number of persons served to allow intensity of service provision. Program goal is to maintain community living by reducing hospitalizations and involvement with judicial system.

Assertive Rehabilitation Treatment (ART) - Is a grant funded by the Texas Council OMNSI program providing intensive rehabilitation services to offenders with a mental illness referred through probation and parole. Services include continuity of care, service coordination, psychiatric services, rehabilitation skills training, individual or group counseling and pre-release screening. Program goal is to reduce involvement with judicial system.

Cognitive Behavioral Therapy - Limited counseling services by licensed staff for short-term counseling needs are available to eligible individuals based on diagnosis and needs assessment.

Continuity of Care - Works with persons who are entering or leaving state or local contracted psychiatric hospital care and assists in the transition back to the community. Family involvement is emphasized. The program goal is to monitor progress during the hospitalization and effectively links the consumer with aftercare services upon discharge.

Project Home - A federal grant funded through the U.S. Department of Housing and Urban Development (HUD) and the City of Corpus Christi to provide housing services targeted toward addressing the homelessness of individuals with mental illness. Funding is provided for rental assistance for up to 2 years for individual who are homeless and have no income. The program requires that a plan for self-sufficiency be developed so that the individual is self-sufficient by the end of the two years.

Inpatient Psychiatric Services - The Center contracts with local hospitals for the provision of adult inpatient and child psychiatric care for individuals without a third-party payer for this type of service. Individuals may also receive services at the San Antonio Ave. Hospital.
PATH (Projects for Assistance in Transition from Homelessness) - A grant program to identify homeless individuals who have a mental illness and to link them to ongoing mental health services. Services include outreach screening, diagnostic assessment, treatment and assistance with housing needs. Staff provide services in the field where homeless individuals can be found.

Robstown Outpatient Clinic - Provides assessments, psychiatric services, psychiatric medications, nursing services, service coordination, rehabilitative services and counseling services for adults. Program goal is to provide services to people that are eligible and residing in the western portion of Nueces County.

Supported Employment - Service activities with the capacity to provide individualized assistance in choosing and obtaining employment, integrated work sites in regular community jobs, and long-term supports provided by identified staff who will assist individuals in keeping employment and/or finding another job as necessary. Services include job readiness, job interview training, job coaching, job search, job development and follow-up with individual and employer. Program goal is to secure and maintain employment for individuals with a mental illness. This service is a part of Rehabilitative Case Management and is also an add-on service for persons eligible for Routine Case Management.

Youth Mental Health Services - An assessment is completed by a licensed professional to access a comprehensive array of behavioral health services that are designed to improve a child's functioning and reduce symptoms. Services are provided to eligible children ages 3 through 17. Services include: assessment, medication clinic, counseling, skills training, case management, family support, crisis intervention and inpatient crisis stabilization.
