Intellectual Disabilities Services

Intake/Assessment/Continuity of Care - The contact point for Mental Retardation Services for individuals ages 3 and up. Assessments are conducted to determine eligibility for services. Additionally, this unit facilitates and monitors discharges from the state facilities, coordinates enrollments for Medicaid Waiver programs, and maintains a waiting list for services.

Robstown Outpatient Clinic - Provides service coordination, psychiatric services, medication, vocational training, counseling, and early childhood intervention services.

Community Supports-Community Training
- Provides individualized activities consistent with the person-directed plan, provided in the home and at various community locations as appropriate. Provides habilitation or support activities to improve or facilitate the ability to perform daily living activities. Includes support services to the family to help preserve the family unit and prevent or limit out-of-home placement.

Respite - Provides short term relief services for the unpaid caregiver when temporarily unavailable to provide supports. Respite also provides personal assistance in daily living activities, functional living tasks, and supervision for safety and security. This service includes habilitation activities, social interaction, participation in leisure activities, and use of natural supports and typical community services available to all people.

Service Coordination
- Service Coordinators use a person-directed process to link, coordinate services, and support people to achieve their personal outcomes. Service Coordination includes writing, reviewing, revising, implementing and monitoring the plan of services and supports. Service Coordinators assist people in securing and maintaining services in the community including education, training, transportation, specialized therapies, advocacy, food, clothing, financial assistance and crisis management. Service Coordination is also provided for individuals enrolled in the Texas Home Living Medicaid waiver program.

In-Home & Family Support (IHFS) - Assists eligible individuals or families to purchase items, services, supports, adaptive devices, or architectural modifications that are related to their mental disability, and necessary to maintain functioning in their own or family home. The actual amount of assistance is based upon the individual's services needs, and application of a co-payment adjusted for income. There is an additional grant for the purchase or lease of special equipment or architectural modifications available for the participants, which may be awarded only once in their lifetime.

Home and Community Based Services (HCS) - Provides support for individuals with mental retardation in a foster care setting. This program offers a full array of services based on the needs of the person, including day habilitation, supported home living, behavioral supports and respite services. This program encourages personal choice and maximum level of independence while using natural supports to achieve goals.

Texas Home Living Services - Services are similar to those offered in the HCS Program except for the residential services.

Specialized Therapies - are assessments and treatment by licensed or certified professionals. These services include medication clinic, nursing services, counseling and behavioral supports.

Advance Employment Service (AES) - Provides services that are designed to meet the vocational goals of each participant. Vocational Training - is provided on site and in the community to enable the individual to obtain employment. Employment Assistance - assists the individual to locate paid individualized competitive employment in the community by helping him/her identify employment preferences, job skills, work requirements and conditions, and prospective employers offering employment compatible with the individual's identified preferences, skills, and requirements. The goal is to match the person's skills with their desired career and develop natural supports in the work environment. Supportive Employment - is competitive employment in an integrated setting with on-going individualized support services consistent with the individual's person-directed plan. The service includes supervision and training, essential to sustain paid competitive work by an individual.
